Support Coordination 

How Support Coordination Fits into Your NDIS Plan:

  • Your NDIS funding is carefully determined based on your unique goals and what's deemed 'reasonable and necessary.' This funding, coupled with support from family, friends, and community services, is at the core of your plan.

  • When necessary and justifiable, Support Coordination can be included in your Capacity Building budget. This allocation provides a dedicated budget for a support coordinator to assist you in navigating your plan successfully.

  • Depending on your plan, the level of Support Coordination may be explicitly specified. If not, you have the flexibility to choose the level that suits your individual needs.

  • Our Support Coordinators understand that every journey is distinct. We tailor our approach to your unique requirements, ensuring that you receive the support that aligns with your goals and circumstances.

 

Our Support Coordinator's key responsibilities are:

  • Understanding Your NDIS Plan:

    • Your Support Coordinator will collaborate closely with you to ensure you have a comprehensive grasp of your NDIS plan. We'll help you navigate support budgets and provide clarity on how to manage your funding.

  • Planning and Coordinating Supports:

    • Together, we'll strategize and identify a mix of services and supports that cater to your specific needs and preferences, all while staying within your NDIS plan budget.

  • Connecting You to Local Services:

    • Our team is well-versed in local services, ensuring you have access to NDIS, community, and government resources that align with your goals.

  • Establishing and Maintaining Supports:

    • We assist you in setting up service agreements, addressing any issues that might hinder your plan's effectiveness, and ensuring you receive the support you need.

  • Coaching, Refining, and Reflecting:

    • Our aim is to equip you with the skills and confidence to independently manage your plan. We'll guide you through NDIS systems, negotiations with providers, and maximising the benefits of your plan.

  • Preparing for Unexpected Events:

    • Together, we'll develop a plan to handle unforeseen circumstances, ensuring that you're well-prepared and supported.

  • Acting in Your Best Interest:

    • Your Support Coordinator always has your best interests at heart. We recommend and provide supports and services that align with your unique needs, always prioritising your well-being.

With over 19 years of dedicated service in the community sector, Kristen brings a wealth of experience and compassion to her work. Her roles have been as diverse as they are impactful, ranging from providing direct support to individuals with disabilities in their homes and communities, to leading support teams in Supported Independent Living (SIL) housing. Kristen has also been instrumental in assisting parents with children living with disabilities who were in out-of-home care, as well as excelling in support coordination.

 

Beyond her professional endeavours, Kristen is a proud and nurturing mother to a blended family of five mostly adult boys, encompassing biological sons, stepsons, and a foster son. This personal journey has granted her invaluable firsthand insight into the unique challenges and triumphs faced by parents of children with disabilities.

 

In her leisure time, Kristen finds joy in quality moments with her family, crafting delicious meals that are savoured and appreciated by all. She is an avid reader, with a penchant for non-fiction works, especially those penned by her Favorite author, Cathy Glass. The beach is Kristen's sanctuary, where she wields her cherished 9FT ALLY  902MH fishing rod, finding solace in the rhythm of the waves and the thrill of the catch.

 

Kristen's home is a lively haven, resonating with the harmonious symphony of her cherished animals. A loyal canine companion, an ageless feline friend, two vibrant and talkative parrots, and a school of five Koi bring vibrant energy to her abode. It's a testament to Kristen's nurturing spirit and her love for all creatures great and small.

 

Rainbow holds a special place in Kristen's heart, reflecting her belief that every colour serves a purpose and holds significance in life's vibrant tapestry. This sentiment mirrors her approach to both her professional and personal life - valuing diversity and finding beauty in every shade.

 

In every facet of her life, Kristen's dedication, warmth, and open-heartedness shine through, making her an invaluable asset to the community sector and a cherished presence in the lives of those she touches.

Tash seamlessly integrates into an additional role within the Blue Step Community, embodying a skill set finely tuned to her role as Coordinator of Support. Drawing from her background as an early childhood educator and her experience as a psychosocial community support worker, Tash's meticulous organisation and professional attributes position her as an invaluable asset to our support coordination team.

Tash is dedicated to building capacity with the people she supports, this is evident in the soothing demeanour she approaches her work, creating an environment of comfort and growth. Tash's innate ability to inspire and provide solace enriches our dynamic as a community, nurturing personal development and well-being.

Tash's expertise extends to working within multidisciplinary teams, where she values collaboration and communication in achieving optimal outcomes for individuals. Proficient in report writing and analysis, she ensures informed decision-making and comprehensive support planning.

Tash is deeply committed to capacity building, Tash empowers individuals to understand their plans and budget reports, fostering autonomy and self-efficacy. Her collaborative approach underscores her dedication to supporting others on their journey towards achieving their goals.

Caite

Masters of Nursing (Paediatrics)

Incontinence and Nursing Assessments

We are thrilled to welcome Caite to our exceptional team! With a background as a Paediatric Registered Nurse, Caite brings invaluable expertise, including the ability to prepare Incontinence Assessments. Holding a Bachelor of Nursing, Graduate Certificate in Clinical Nursing (Paediatrics), and Master of Advanced Practice Nursing (Paediatrics), Caite offers a wealth of knowledge cultivated across diverse healthcare environments in Darwin and SE QLD.

 

Caite embarked on her journey in healthcare, transitioning from the acute mental health ward to NICU/SCN in Darwin. her passion for paediatrics blossomed through exposure to various medical, surgical, and infectious disease wards. Specialising in paediatric emergency departments and gaining experience in general medical/surgical settings, Caite now contributes her expertise to paediatric intensive care, adeptly managing a spectrum of medical and surgical conditions. Caite's skill set is extensive, encompassing critical procedures below.
